* Please Note:
All New Employees must complete the above forms in order to begin employment. One form must be done for every employer where you have had direct contact with children*. This includes schools, daycares, summer camp, etc. An employee may not begin working until all verification forms are completed. Candidates are strongly encouraged to complete the forms in advance and submit the forms with their application and resume.
*Per PDE, Direct contact with children is defined in § 6303 (relating to definitions) as the care, supervision, guidance or control of children or routine interaction with children.
